10/17/2023 EnchantaVaughn Approved Behavior: Upon arrival, client behaviors were viewed as chatty, fidgety, and anxious. Client was not in a good mood. Client displayed uncontrollable behaviors, as he repeatedly laid on the floor, rolling around, and kicking furniture. Client attempted to push his brother and sister when they teased him for his behaviors. Client lashed out at his mom without any form of remorse for his negative and nonchalant attitude. Client was reprimanded by his counselor on the appropriate ways to carry himself. Behavior: Client is often observed showing frequent rage, in which he exhibited oppositional defiance, failing to consider how it would make his loved ones feel. Client was non-compliant for most of the session. It was difficult for client to remain in his seat while debriefing with QMHP. Client appeared very moody today for no reason neglecting his duties in the home and in the session. Client attitude did not change, as he proceeded to answer QMHP questions with short and quick responses. Client did not seem interested in working towards a better mental and spiritual well-being. Intervention: QMHP engaged the client in conversation to get his mind off things. QMHP practiced calming techniques by modeling examples for the client to follow, such as walking away and cooling off and counting to ten when he found himself stuck in his feelings. QMHP assisted client with a series of calming imageries to reduce stressors and uplift client when he was down and out. QMHP inquired with client his feelings, moods, and behaviors and what he can do to make his life easier. Intervention: QMHP observed the client’s digression and responded accordingly. QMHP reiterated the directives 3 times to ensure the client understood the exercise. QMHP encouraged the client to try to release his emotions so that he doesn’t bottle them up and become discouraged. QMHP prompted the client to think about how his negative actions can affect others. QMHP reminded the client that talking about his feelings is healthy and a natural part of therapy. Response: Client utilized effective communication and assertiveness skills to express his anger more calmly and safely. Client displayed low participation and low confidence as he processed with QMHP. Client did not implement positive “I” statements but instead made inappropriate comments and refused to follow along with his individualized service plan. Client displays challenges in a few areas, including but not limited to his mental health which have been addressed. Response: Client continues to make poor choices in session as it pertains to verbal skills and communication. Client stated that he has a hard time controlling his thoughts and words and speaks his mind even if it hurts someone feelings because a lot of his anxieties exist from other people saying mean things to him or teasing him for his lack of emotional intelligence. Client agrees to journaling his feelings, as he reflects on actions and consequences.
